Street Medicine Team RN

Janian Medical Care, P.C., seeks full time Registered Nurse for the Street Medicine Teams in Manhattan. Janian Medical Care, P.C., an affiliate of Center for Urban Community Services (CUCS) seeks a NYS Licensed Registered Nurse to join our Division of Nursing Services. Janian Medical Care provides primary medical, psychiatric, and nursing care to homeless and formerly homeless New Yorkers at community based programs including supportive and transitional housing, shelters, and homeless outreach teams. We are seeking a dynamic, collaborative Registered Nurse, who is dedicated to working with our complex and marginalized patients in non traditional settings.

Due to the success Janian's Street Medicine program has had in providing quality primary care services to people experiencing street homelessness, the Street Medicine Teams were expanded in recent years to twice the number of teams in Manhattan, Brooklyn, and Queens. The Street Medicine Teams work with people for as long as is necessary with a focus on quality of care provided over quantity of clients served. Each team consists of 1 NP/MD and 1 RN with a Van Coordinator to support operations. The teams work closely with outreach teams who identify and refer medically vulnerable clients.

The Registered Nurse will have primary responsibility for improving the overall health of program participants including promotion of physical and mental health integration. The Registered Nurse will work with members of the multi-disciplinary Street Medicine Team and our outreach teams towards this goal by engaging in the following activities:

  • Conducting diagnostic and risk assessments and providing care in multiple settings including in institutional settings, participants’ homes, our medical suites, and on the streets
  • Providing clinical care, including administering medication and performing wound care, at the direction of the Street Medicine and onsite Primary Care Providers
  • Working collaboratively with patients to identify and overcome barriers to treatment
  • Linking participants to primary and specialty care • Participating in multidisciplinary team meetings
  • Liaising with pharmacies, hospitals and city agencies as needed

Current Massachusetts licensure is required, maintenance of DEA number and Mass. DPH registration to prescribe controlled substances. Knowledge of a second language is preferred.

Medical Director

The Village Health Center of Father Joe's Villages in San Diego is seeking a primary care physician to serve as Medical Director. The Health Center is located on the campus of Father Joe's Villages. Established in 1950 to serve San Diegans experiencing homelessness and poverty, Father Joe’s Villages has grown to include a comprehensive campus and scattered-site programs that house over 2,000 people nightly. It all began with a small chapel in downtown San Diego. Every day, the Village Health Center provides the highest quality healthcare to our homeless and at-risk neighbors and supports our community in ending homelessness one life at a time.

The medical director provides clinical leadership and oversight within a Federally Qualified Health Center serving patients who identify as homeless or are at-risk of homelessness. This position is responsible for management of clinical staff for the Health Center. This position directly manages the Manager of Clinical Services, Addiction Medicine Program Lead, Psychiatry Program Lead, and a team of primary care and psychiatry providers (including MDs, DOs, and APPs). As a clinician leader, this position provides direct primary care services to patients of the Village Health Center (VHC).

UPMC Street Medicine Fellowship

The UPMC Mercy Street Medicine Fellowship is the first of its kind. It is a program of the UPMC Mercy Hospital Department of Internal Medicine in Pittsburgh, Pennsylvania. The Fellowship was created in collaboration with Pittsburgh Mercy’s Operation Safety Net which serves as the primary arena for the street medicine experience. A particular emphasis of the Fellowship is the interface of care between the unsheltered population and the hospital systems of Pittsburgh.  These areas include the Emergency Medicine Department (UPMC Mercy has the highest volume ED in the city), the inpatient street medicine consult service, outpatient referral and care (the Fellow will have a continuity clinic at UPMC Mercy), the Respite Care Center (for homeless patients who require medical recovery) as well as a focus on patient navigation throughout the larger health system of the city.
